Insurance
We accept most major insurance plans as well as most auto and worker's compensation.
Appointments
All office visits are by appointment only and must be made in advance. You may contact our office to make an appointment by calling 302-478-7001, option 3. An appointment will be made once you provide us with your:
• Contact Information (Name, Address, Phone Numbers, Etc.)
• Insurance Information
• Reports from Any Tests You've Had Done Related to Your Pain*
• Medical Notes from Your Doctor Who Has Been Treating Your Pain so Far*

When you come to your initial consultation visit, please bring your insurance cards, auto or worker's compensation information if applicable, a picture ID, and a list of all your current medications. Without this information, you cannot be seen. In addition, please arrive 30 minutes prior to your scheduled appointment time to complete our registration and history forms; if you are able to download and complete the forms prior to your appointment then you will only need to arrive 15 minutes before your scheduled time.
For existing patients, please inform our staff of any changes to your insurance, address, or contact 302-478-7001
when you make your appointment. Please bring your new insurance card(s) with you to your appointment. Failure to provide these changes will misdirect your bills and can cause you to be directly responsible for payment.
Cancelling
We ask that you provide at least 24 hours' notice if you are unable to make your appointment. This will allow us to use that time to see another patient who needs our services. Repeatedly missing appointments does not allow us to care for you properly and therefore may cause us to terminate our relationship with you.
Payments
Co-pays are expected at the time of your visit. For your convenience, we can accept cash, check, and credit card payments.
